29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) Thermoforming, Line 1 - On or about May 14, 2025, the employer failed to provide effective guarding to protect employees working at or near the Line 1 Former Machine. An employee sustained a right middle finger amputation while adjusting a sheet guide on the Line 1 Former Machine when the employee's right hand was caught between the pin chain and the idler wheel. The employee was exposed to ingoing nip point and caught-in hazards.
