29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level: a) Refuge Plaza East, 37427 Gateway Lane, Unit A, Selbyville Delaware 19975: On or about August 27, 2024, an employee installing windows while providing force to the lower left corner fell from an extension ladder that was not set on a stable, level surface which resulted in the employee's hospitalization. The surface contained concrete wash out material that was allowed to dry unevenly.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways: a) Refuge Plaza East, 37427 Gateway Lane, Unit A, Selbyville Delaware 19975: On or about August 27, 2024, the employer did not provide training to employees working on or with various types of ladders to gain access to their work area on elevated surfaces.
