29 CFR 1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) Kane Innovations, Inc. on or about August 19, 2024 at the address of 515 North Fraley Street: An employee was operating the Wysong shear in the k1 building. The shear was not guarded on the front side of the shear, resulting in a fingertip amputation. B) Kane Innovations, Inc. on or about August 27, 2024 at the address of 515 North Fraley Street: The Wysong shear is not guarded on the backside of the shear. C) Kane Innovations, Inc. on or about August 27, 2024 at the address of 515 North Fraley Street: The hold downs on the Wysong shear were not guarded. Exposed employees to a pinch point/amputation hazard.

OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees operating the Wysong shear was not protected from unintended and/or accidental operation or cycling resulting in fractures and amputations. A) Kane Innovations, Inc. on or about August 27, 2024 at the address of 515 North Fraley Street: The Wysong shear activating foot pedal crossbar was not protected to prevent accidental cycling of the shear.
