Sugar St. Lima, Ohio 45801:  The employer did not ensure employees tasked with performing lockout covered tasks followed established procedures in sequence to ensure equipment was fully deenergized. Due to this condition, employees tasked with service and maintenance of Line 400, UBE bagger number four (Model 1216AL/SN 1116) were exposed to caught in hazards created between the rotating sprocket and chain of the discharge conveyor.   Alfred Nickle's Bakery, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d), which was contained in OSHA inspection number 1669695, citation number 1, item number 3. and was affirmed as a final order on January 5, 2024, with respect to a workplace located at 1950 N.
