29 CFR  1926.200(g)(2): Traffic control signs or devices used for protection of construction workers did not conform to Part VI of the Manual of Uniform Traffic Control Devices (AMUTCD"), 1988 Edition, Revision 3, September 3, 1993, FHWA-SA-94-027 or Part VI of the Manual on Uniform Traffic Control Devices, Millennium Edition, December 2000, FHWA.  On or about September 11, 2024, employees were exposed to struck-by hazards at the intersection of SR 305 and SR 20, Wickliffe OH, 44092, where the employer did not have the following traffic control signs or devices in use to protect the crew that had two out of four lanes of traffic closed for an asphalt patching operation per 6H-37, 6F.61 and Table 6C-3 of the MUTCD.  a) The employer did not ensure advanced warning signs used for traffic control were properly placed for a double right lane closure on a multilane highway, per the MUTCD.  b) The employer did not ensure a second arrow board used for traffic control was utilized for a double right lane closure on a multilane highway, per TA 6H-37 of the MUTCD.  C) The employer did not ensure an adequate merging taper was installed for a double lane closure on a multilane highway, per TA 6H-37, Table 6C-3 and Section 6G.12 of the MUTCD.  AWP, Inc, was previously cited for a violation of this occupational safety and standard 29 CFR 1926.200(g)(2), which was contained in OSHA inspection number #1736165, citation number 2 item 1 and was affirmed as a final order on 6/3/2024, with respect to a workplace located at 5610 Wilson Mills Rd., Highland Heights, OH, 44013.
