29 CFR 1926.200(g)(1): Construction areas were not posted with legible traffic signs at points of hazards: a) W. Shelley Dr./Darley Rd., Claymont, DE 19703 (eastbound): On or about September 24, 2024, the employer failed to provide flagger ahead signs visible to the public while traveling eastbound on Darley Road to assist traffic in understanding traffic pattern changes at the point of hazard due to utility construction and flagging operations as stated in the MUTCD Section 6C.04 This exposed employees to multiple hazards onsite including but not limited to struck by/ caught in hazards when they performed flagging operations. The AWP, Inc. dba Area Wide Protective was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.200(g)(1) which was contained in OSHA inspection number 1736165, citation number 1, item number 3 and was affirmed as a final order on 6-3-2024, with respect to a workplace located at 5619 Wilson Mills Rd. Highland Heights, Ohio 44143.

Shelley Dr./Darley Rd., Claymont, DE 19703: On or about September 24, 2024, employees were exposed to multiple hazards onsite including but not limited to struck by/ caught in hazards when they performed flagging operations activities. Road signage used did not conform to provide protection for the equipment and employees. Company vehicles and employees were observed exposed when the road signage was not positioned as required by the MUTCD Section 6F.17. b) a) W. Shelley Dr./Darley Rd., Claymont, DE 19703: On or about September 24, 2024, employees were exposed to multiple hazards onsite including but not limited to struck by/ caught in hazards when they performed flagging operations activities. Road signage used did not conform to provide protection for the equipment and employees. Shelley Dr./Darley Rd., Claymont, DE 19703: On or about September 24, 2024, employees were exposed to struck by hazards when they worked in live lanes of traffic and stopped vehicular traffic using hand signals alone, employees were not using the required slow/stop paddle as required in the MUTCD 2009 edition 6E .07. b) W. Shelley Dr./Darley Rd., Claymont, DE 19703: On or about September 24, 2024, employees were exposed to struck by hazards when they stood in active roadways with her back turned to traffic and failed to follow flagger procedures in section 6E .07 of the 2009 Edition of the MUTCD.
