(a) On or about October 1, 2024, at 10310 South Shepard Avenue, Oak Creek, WI 53154, employees installing a roof on an existing residential home roof with an approximately 7:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately nine (9) feet from the roof eve to the ground below on the front and back sides of the home. German Gutierrez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1457971, citation number 1, item number 1 and was affirmed as a final order on June 12, 2020, with respect to a workplace located at 501 Samuels Ave., Fort Worth, TX 76102.
