29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  (a) On or about March 13, 2025, employees conducting re-roofing on a residential home with a 4:12 pitch roof were not using fall protection. This exposed employees to a fall hazard of approximately 13 feet from the roof eve to the ground below on the back and front of the house.  (b) On or about March 13, 2025, employees conducting re-roofing on a residential home with a 4:12 pitch roof were not using fall protection. This exposed employees to a fall hazard of approximately 9 feet from the roof eve to the ground below on the back and front of the house.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  (a) On or about March 13, 2025, the employer did not ensure that eye protective equipment was used when employees were exposed to eye injury from flying projectiles while using pneumatic nail guns.
