29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652. Site: 21-07 Astoria Blvd, Long Island City, NY 11102. a). On or about September 30, 2024, ASF Construction & Excavation Corp. employees engaged in formwork on the foundation wall of a new building were exposed to loose soil in an excavation that was not shored. This hazard exposed employees to being crushed in between the excavation and foundation wall.

On or about September 30, 2024, ASF Construction & Excavation Corp. employees engaged in foundation construction of a new building were exposed to impalement hazard from rebar that were not capped or bent.
