29 CFR 1910.29(b)(2): The employer did not ensure that guardrail systems met the requirement that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members that are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high. At a worksite located at 5302 Stockyards Expressway, Saint Joseph, MO 64504, employees were exposed to fall hazards. The employer exposed the employees to these hazards by allowing them to access an elevated platform in the AMR section of the facility that was not equipped with a mid rail at the point of access where a fixed ladder was used to access the platform.

29 CFR 1910.305(a)(1)(i): Metal raceways, cable armor, and other metal enclosures for conductors were not connected to all boxes, fittings, and cabinets to provide effective electrical continuity: At a worksite located at 5302 Stockyards Expressway, Saint Joseph, MO 64504, employees were exposed to electrical hazards that include burns, shocks, arc flash and fire. The employer exposed the employees to these hazards by allowing them to work near exposed electrical wire after the conduit that housed them became detached. Instances of exposure include, but are not limited to: 1. Conduit housing electrical wires on the south bone cannon machine, near the southern wall of the AMR area had become detached, exposing electrical wires. 2. Conduit housing electrical wires near an elevated platform in the northern section of the AMR area had become detached, exposing electrical wires.

29 CFR 1910.305(b)(2)(i):Pull boxes, junction boxes, and fittings were not provided with covers approved for the purpose: At a worksite located at 5302 Stockyards Expressway, Saint Joseph, MO 64504, employees were exposed to electrical hazards that include burns, electrocution, shocks, arc flash and fire. The employer exposed the employees to these hazards by allowing them to work near electrical junction boxes that did not have appropriate covers. Instances of exposure include, but are not limited to: 1. Employees in the AMR section were exposed to electrical hazards while working near an electrical junction box with a cover that did not fully enclose the internal electrical wires. The box was located on the southern wall of the AMR section, East of the inclined conveyor. 2. Employees in the AMR section were exposed to electrical hazards while working near an electrical junction box with a cover that did not fully enclose the internal electrical wires. The box was located on the southern wall of the AMR section, West of the inclined conveyor.
