29 CFR 1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system: (a) 304 at Valley Point, New Commercial Construction, 4th Floor Roof Trusses: On October 8, 2024 employees were working within the trusses approximately 9 feet above the floor without the use of a fall protection system in place.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. (a) 304 at Valley Point, New Commercial Construction, 4th Floor Roof Trusses: On October 8, 2024 an employee was working in the trusses using a nail gun without the use of safety glasses.
