(a) On or about October 10, 2024, at 10301 S. Ashley Ln, Oak Creek, WI 53154, the employer did not ensure employees conducting installation of a new residential roof, using hand tools and nail guns were wearing eye protection. This exposed the employees to struck by hazards and eye injuries. Top Quality Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.102(a)(1), which was contained in OSHA inspection number 1667388, citation number 1, item number 3 and was affirmed as a final order on August 17, 2023, with respect to a workplace located at 18 W Sedgemeadow St, Elkhorn, WI 53121.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about October 10, 2024, at 10301 S. Ashley Ln, Oak Creek, WI 53154, employees installing a roof on an existing residential home with an approximately 6:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 17 feet 2 inches from the roof eve to the ground below on the back (west) side of the home. Top Quality Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1667388, citation number 1, item number 1 and was affirmed as a final order on August 17, 2023, with respect to a workplace located at 18 W Sedgemeadow St, Elkhorn, WI 53121.
