29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s) a) 4100 Mullan Rd Missoula, MT 59808: On or about October 8, 2024 and at times prior, the General Contractor, Rhino Roofing, LLC, failed to ensure its sub contractor, HD Roofing, protected their employees from fall hazards greater than 25 feet to the ground below while performing roofing operations.
