On or about September 18, 2025, on the roof of a residential reroofing project, an employee and employees of subcontractors were not in use of fall protection while performing roofing work activities from a roof that was more than six (6) feet from the ground.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about September 18, 2025, on the roof of a residential roofing project, an employee and employees of subcontractors performing roofing work activities accessed the roof using ladders with siderails that did not extend at least three (3) feet above the landing.
