Condition noted  on or about 10/17/24.  b - Front of Structure, Community Building, 900 Gary Smith Way, Forked River NJ:  A worker was exposed to fall hazards, in excess of 14 feet 7 inches, when partially working from an unsecured, aluminum extension ladder (one foot on the ladder and the other on the commercial structure's steep pitched, upper roof) while not utilizing fall protection.  Condition noted  on or about 10/17/24.  NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM. FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
