29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a) On October 22, 2024: employees operated a Unicarriers MCU1f2A25LV forklift equipped with a push pull front-end attachment. The forklift was listed as a side shifter as the attachment and did not list the push-pull attachment. This attachment is used to load and unload product weighing between 2,000 - 2,500 pounds from the shipping trucks. Idaho Pacific Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1910.178(a)(5)), which was contained in OSHA inspection number 1677876, citation number 1-1-b and was affirmed as a final order on 11-21-2023, with respect to a workplace located at 4723 East 100 North RIRIE, ID 83443.
