Operation of pneumatic nail guns can cause significant eye injury including severe vision loss and blindness. Nail gun injuries can cause severe ocular trauma directly, via ricochet after striking a hard surface or metal, or the gun might backfire or jam.

The personnel basket lacked a front guardrail and employees did not utilize personal fall arrest systems when in the basket. This condition exposed employees to falls up to 37 feet above the ground.
