29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) On or about November 7, 2024, and prior, Buffer 10111 did not have adequate guarding at the operators position to prevent employee access to the rotating mandrels and moving parts, such as the buffers on each of the mandrels during normal production operations and cleaning operations, exposing employees to crushing, caught-in and amputation hazards. (b) On or about November 7, 2024, and prior, Buffer 10413 did not have adequate guarding at the operators position to prevent employee access to the rotating mandrels and moving parts, such as the buffers on each of the mandrels during normal production operations and cleaning operations, exposing employees to crushing, caught-in and amputation hazards. (c) On or about February 10, 2025 and prior, Rotary Sander 11670 did not have adequate guarding to prevent employee access to rotating and moving parts such as the rotating table of the rotary sander and other moving parts during production operations, exposing employees to crushing, caught-in and amputation hazards. (d) On or about February 10, 2025 and prior, Rotary Sander 11709 did not have adequate guarding to prevent employee access to rotating and moving parts such as the rotating table of the rotary sander and other moving parts during production operations, exposing employees to crushing, caught-in and amputation hazards. (e) On or about February 10, 2025 and prior, Rotary Sander 11732 did not have adequate guarding to prevent employee access to rotating and moving parts such as the rotating table of the rotary sander during production operations, exposing employees to crushing, caught-in and amputation hazards. (f) On or about February 10, 2025 and prior, Rotary Sander 11760 did not have adequate guarding to prevent employee access to rotating and moving parts such as the rotating table of the rotary sander during production operations, exposing employees to crushing, caught-in and amputation hazards.
