29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  a) On or about 11/12/2024, in the tool room;  Employees were exposed to struck by/caught in hazards when Lockout/Tagout procedures were not developed, documented, and utilized for the purpose of controlling hazardous energy when employees performed servicing and maintenance such as, but not limited to, changing blades on the Jet 8x14 horizontal bandsaw.  b) On or about 11/12/2024, in the Perforating Department;  Employees were exposed to struck by/caught in hazards when Lockout/Tagout procedures were not and utilized for the purpose of controlling hazardous energy when employees performed servicing and maintenance such as, but not limited to, changing and setting up dies on the Soenen 604 Press.
