29 CFR 1910.147(c)(6)(i): The employer did not conduct an annual or more frequent inspection of the energy control procedure to ensure that the procedure and requirements of this standard were followed:  a) On or about November 15, 2024, the employer did not ensure periodic inspections of the energy control procedures for the compound polyethylene bubble film making machine were completed at least annually. Employees working on the compound polyethylene bubble film making machine are required to adjust the shoe, vacuum lines,  and perform repairs and/ or adjustments to the chain system which requires the use of energy control procedures that have not been reviewed for deficiencies, thereby exposing employees to caught in and crushed by hazards.   b) On or about November 20, 2024,  the employer did not ensure periodic inspections of the energy control procedures for the green grinder on the repo line were completed at least annually. Employees working on the grinder are required to perform maintenance and/ or service tasks such as unjamming foam from the grinder which requires the use of energy control procedures that have not been reviewed for deficiencies, thereby exposing employees to caught in and crushed by hazards.
