OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire and explosion hazards associated with the accumulation of flammable isobutane gas. International Cushioning Company, LLC located at 2101 Cedar Street in Fremont, Ohio; On or about February 25, 2025, the employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees recycling scrap foam in the re-grinders on the recycling line were exposed to fire and explosion hazards associated with the accumulation of flammable isobutane gas above the lower explosive limit.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: International Cushioning Company, LLC located at 2101 Cedar Street in Fremont, Ohio; On or about February 25, 2025, the employer did not ensure employees who were required to load the polystyrene foam into re-grinders were provided with flame retardant clothing. Employees feeding scrap foam sheets into the re-grinders (red and green) were exposed to fire and explosion hazards when levels of isobutane exceed the lower explosion limit.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: International Cushioning Company, LLC located at 2101 Cedar Street in Fremont, Ohio; Instance A On or about February 25, 2025, the employer did not ensure that flexible cables were not used as a substitute for fixed wiring. Employees were exposed to electrical shock and fire hazards in that a flexible extension cord was used as fixed wiring to power electrical equipment such as, but not limited to, the Takk Static Elimination Blower. Instance B On or about February 25, 2025, the employer did not ensure that flexible cables were not used as a substitute for fixed wiring. Employees were exposed to electrical shock and fire hazards in that a flexible extension cord was used as fixed wiring to power electrical equipment such as, but not limited to, an overhead light on the recycling line above the hopper and conveyor.
