29 CFR  1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, Items A-D of this section:  a) On or about November 24, 2024, the employer failed to clearly identify all of the specific steps to be followed by employees to isolate and disable components associated with the No. 1 Extruder Line in order to safely de-energize the equipment and control it's hazardous energy. Energy control procedure LOTO-Extruder 1 lacked specific steps to relieve hazardous hydraulic and pneumatic energy and to dissipate hazardous thermal energy prior to equipment access. The procedure did not contain clear and specific verification steps to determine the effectiveness of the energy control measures. Additionally, it did not address all hazardous energy, such as gravity and the potential for unexpected lowering of rollers. Employees were exposed to caught-in and amputation hazards associated with, but not limited to, the equipment's spinning vertical chrome rollers when performing service and maintenance activities such as, but not limited to, adjustments, changes, and threading on Extruder No. 1 components, when the equipment was not de-energized.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

The equipment was not de-energized which exposed employees to caught-in and amputation hazards associated with in-running nip points and unguarded powered rollers when threading rollers with their hands while the rollers were spinning.  As a result, the remaining applicable energy control elements, involving machine isolation [1910.147(d)(3)], lockout/tagout device application [1910.147(d)(4)], dissipation of residual energy [1910(d)(5)(i)], and verification of isolation [1910.147(d)(6)] were not implemented to protect employees from machine hazards.  Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).
