29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket. On or about December 18, 2024, and at times prior to, on the east side of the jobsite (front of the building), employee(s) utilized an aerial lift and stood on the mid-rail of the aerial lift while performing welding work on the exterior of the building. Warrior Steel, LLC., was previously cited for a violation of this Occupational Safety and Health standard, 29 CFR 1926.453(b)(2)(iv), which was contained in OSHA inspection number 1669344, citation number 01, item number 01, and was affirmed as a final order on August 3, 2023, with respect to a workplace located at 300 N. Cimarron, Tuttle, OK 73089.

On or about December 18, 2024, and at times prior to, on the east side of the jobsite (front of the building), employee(s) in an aerial lift did not wear fall protection with lanyards attached to the boom or basket while performing welding work on the exterior of the building. Warrior Steel, LLC., was previously cited for a violation of this Occupational Safety and Health standard, 29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 1559500, citation number 01, item number 01, and was affirmed as a final order on January 7, 2022, with respect to a workplace located at 10201 S. Western, Oklahoma City, OK 73139.
