29 CFR 1926.20(b): Accident prevention responsibilities.  29 CFR  1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.  a) Located at 5665 Hoover Rd, Grove City, OH 43123: On or about December 30, 2024, the employer did not conduct frequent and regular inspections of the job site. As a result, employees were exposed to fall hazards greater than 25 feet while performing residential construction.

29 CFR  1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.  a) Located at 5665 Hoover Rd, Grove City, OH 43123: On or about December 30, 2024, the employer did not ensure that employees framing were protected from falling by a fall protection system. As a result, employees were exposed to a fall hazard of greater than 25 feet while performing commercial construction.

29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:  a) Located 5665 Hoover Rd, Grove City, OH 43123: On or about December 30, 2024, employees were installing roofing material on a new commercial facility without the use of proper anchorage, thereby exposing employees to fall hazards of more than 25 feet.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  a) Located at 5665 Hoover Rd, Grove City, OH 43123: On or about December 30, 2024, the employer failed to ensure that each employee exposed to a fall hazard was trained on the hazards of falling and the procedures to minimize the hazards in the work area. As a result, employees were exposed to fall hazards greater than 25 feet while performing commercial construction.
