29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  On or about January 30, 2025, during a construction project on the roof of a commercial building located at 4849 Acorn Dr. Independence, Ohio 44131, the employer did not ensure that employee(s) were protected from possible danger of eye injuries from impact and/or flying debris by operating pneumatic nail guns without the use of eye protection.

29 CFR  1926.501(b)(11): Steep roofs: Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toe boards, safety net systems, or personal fall arrest systems:  On or about January 30, 2025, during a construction project on the roof of a commercial building located at 4849 Acorn Dr. Independence, Ohio 44131, the employer did not ensure employees working from a steep-sloped roof were protected from a fall of approximately 10 feet, 11 inches by the use of a guardrail system, safety net system, personal fall arrest system and/or combination warning line system and safety monitoring system.    Pine Ridge Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(10) Fall Protection), which was contained in OSHA inspection number 1549680, citation number 2, item number 1 and was affirmed as a final order on (10/3/2021), with respect to a workplace located at 10602 W. Pleasant Valley Rd, Param, OH 44130.  Pine Ridge Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Fall Protection), which was contained in OSHA inspection number 1486870, citation number 1, item number 1 and was affirmed as a final order on (10/9/2020), with respect to a workplace located at 33141 Beverly Lane, Avon Lake, Ohio, 44012.
