29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about April 22, 2025, employees were exposed to struck by hazards, while performing roofing work on a residential home located at 2992 Villa Glen Cir NW, Massillon Ohio 44646; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material using air driven nail guns.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about April 22, 2025, employees were exposed to fall hazards, while performing roofing work on a residential home located next to 2992 Villa Glen Cir NW, Massillon Ohio 44646, the employer did not ensure employees working on a residential roof were protected from a fall of approximately 11.1 feet by utilizing a fall protection system.  Pine Ridge Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to use Fall Protection), which was contained in OSHA inspection number 1549680, citation number 02, item number 01 and was affirmed as a final order on (10/13/2021), with respect to a workplace located at 10602 W. Pleasant Valley Rd, PARMA, OH 44130.  Pine Ridge Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to use Fall Protection), which was contained in OSHA inspection number 1516565, citation number 01, item number 01 and was affirmed as a final order on (4/20/2021), with respect to a workplace located at 10015 Rosalee LN, Strongsville Ohio 44136.  Pine Ridge Roofing LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to use Fall Protection), which was contained in OSHA inspection number 1486870, citation number 01, item number 01 and was affirmed as a final order on (10/9/2020), with respect to a workplace located at 33141 Beverly LN, Avon Lake Ohio 44012.
