29 CFR 1926.102(a)(1):The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about April 11, 2025, at 14923 Holland Drive, Brook Park, OH, 44142, the employer failed to ensure employees utilizing pneumatic nail guns to attach weatherproofing to a roof was protected by eye/face protective devices in that employees were wearing no face protection.

29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about April 11, 2025, at 14923 Holland Drive, Brook Park, OH 44142, the employer failed to ensure employees working from a 4:12 pitch roof approximately 13 feet high installing weatherproofing were protected from a fall in that no personal fall arrest systems/devices were utilized.
