29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents: On or about February 5, 2025, during the installation of siding material at a job site located at Lot 637, 514 Giant TR, Seville Ohio 44273; the employer did not ensure that employees installing siding on a residential structure were protected from flying debits while installing siding material with air driven nail guns and hand tools. The employees were actively engaged installing material and did not utilize eye and or face protection.

29 CFR 1926.451(e)(1):When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Crossbraces were used as a means of access. On or about February 5, 2025, during the installation of siding material at a job site located at Lot 637, 514 Giant TR, Seville Ohio 44273; the employer did not ensure an employee working from a ladder jack scaffold had safe means of access/egress by utilizing a portable ladder positioned on the outside of the working platform so the employee would not have to crawl/climb over the working platform to gain access/egress.
