29 CFR 1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed: On or about April 17, 2025, during the installation of siding material at the job site located at 181 Cumberland, Seville Ohio 44273; during the installation of siding material on a new construction residential structure, the employer did not ensure that employees utilized portable ladders (self-supporting) were properly utilized. An employee was observed utilizing a self-supporting ladder collapsed and leaning up against the structure to gain access to an elevated working height.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about April 17,2025, during the installation of siding material at the job site located at 181 Cumberland, Seville Ohio 44273; the employer did not ensure employees working on the roof of a new construction residential structure were protected from a fall of approximately 10.5 feet by utilizing a fall protection system. MODERN EXTERIOR SOLUTIONS LLC DBA Homestead Exterior Solutions was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to have fall protection), which was contained in OSHA inspection number 1800403, citation number 01, item number 02 and was affirmed as a final order on 3/27/2025, with respect to a workplace located at 3479 Alder Ave, Uniontown Ohio 44685.
