On or about February 11, 2025, at 909 W Oak Ridge Rd, Orlando FL: employees were exposed to struck-by hazards while working below roofing activities, in that, employees were not wearing hardhats.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a. On or about February 11, 2025, at 909 W Oak Ridge Rd, Orlando FL: employees were exposed to a 16 foot 8 inch fall hazard while conducting roofing activities on a 4:12 pitched roof, in that, fall protection was not used.  Kerem Services Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(11), which was contained in OSHA inspection number 1615823, citation number 1, item number 1 and was affirmed as a final order on 03/03/2023, with respect to a workplace located at 10649 Bastille Dr. Orlando, FL.

On or about February 11, 2025, at 909 W Oak Ridge Rd, Orlando FL: employer did not prepare and maintain a written fall protection training program certification for employees exposed to fall hazards of 16-foot 8 inches while working on a 4:12 pitch roof.
