29 CFR 1910.178(a)(5):The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: a. On or about May 30, 2025, at the worksite: employees were exposed to an approximate 30 foot fall hazard in that, a job-made wood box was attached to a forklift and used to transport employees and subcontractors from the ground to the work area on the roof.

On or about May 30, 2025, at the worksite: employees were exposed to an approximate 30 foot fall hazard while installing roofing on a 5:12 pitched roof, in that, fall protection was not used. Kerem Services Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard1926.501(b)(13), which was contained in OSHA inspection number 1803566, citation number 2, item number 1 and was affirmed as a final order on 3/12/2025, with respect to a workplace located at 909 W Oak Ridge Rd, Orlando, FL.
