29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, liquid chemicals, acids or caustic liquids, chemical gasses or vapors:  At the jobsite located at 1705 S. Webb Road, Wichita, KS 67207, employees engaged in masonry work were exposed to hazards of splashing mortar, cement, brick, stone, and block pieces in that eye protection was not utilized. Employees were permitted to conduct bricklayer activities and cut brick materials without being required to wear high impact resistant safety glasses while performing masonry work duties.

29 CFR 1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects:  At the jobsite located at 1705 S. Webb Road, Wichita, KS 67207, employees were exposed to struck-by hazards. Employee was observed placing the material boxes from the ground level to the working platform without wearing hard hats. Employees were permitted to walk and work underneath the occupied scaffold platforms and were not protected from falling objects, including but not limited to material and tools that could fall on employees in that toeboards, screens were not installed.

29 CFR  1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  At the front side of the new building located at 1705 S. Webb Road, Wichita, KS 67207, employees engaged in masonry activities working from a tubular welded frame scaffold were exposed to hazards of instability and collapse of the scaffold. Employees were accessing the 7 feet 8 inches high work platforms by climbing up the walk through scaffold end-frames in lieu of using a ladder. Employees were permitted to climb the fabricated scaffold frames and cross braces to access and egress the scaffolding platforms.

Webb Road, Wichita, KS 67207, employees engaged in masonry activities on an active tubular welded frame scaffold were exposed to hazards of struck-by, instability and collapse of the scaffold in that scaffold system was not inspected by a competent person prior to the start of the work shift to identify deficiencies.

Webb Road, Wichita, KS 67207, employees engaged in masonry bricklayer activities on an active tubular welded frame scaffold were exposed to hazards of instability and collapse of the scaffold in that the cross-brace was not connected or joined by coupling or stacking pins to secure and to prevent collapse of the scaffold.
