29 CFR 1910.147(c)(4)(i):Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) On or about March 13, 2025, specific procedures for the control of hazardous energy was not developed and enforced by the employer. Employees assigned to High Pressure Laminate Room and who operated equipment such as but not limited to the OSAMA Automatic Roller Glue Machine (ITALPRESSE, S4R/1600) were not protected from potential caught in and crushing hazards when completing daily cleaning activities. Hazardous energy used at the facility includes but are not limited to electricity and pneumatic energy.
