Citation 1, Item 1 a serious violation of 29 CFR 1926.102(a)(1) with an abatement date of April 29, 2025. Citation 2, Item 1 an other-than-serious violation of 29 CFR 1926.503(b)(1) with an abatement date of April 29, 2025. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATIONS OF THE CORRECTIVE ACTION WORKSHEET).

This exposed the employees to struck by hazards and eye injuries.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b). (a) On or about March 14, 2025, at 600 E. Quail Run, Oak Creek, WI 53154, employees installing a roof on a residential home roof with a 7:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 17 feet 5 inches from the roof eve to the ground below on the back (North) side of the home. (b) On or about March 14, 2025, at 600 E. Quail Run, Oak Creek, WI 53154, employees installing a roof on a residential home roof with a 8:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 8 feet 3 inches from the roof eve to the ground below on the front (South) side of the home.

Quail Run, Oak Creek, WI 53154, employees were accessing the roof to conduct roofing work from a ladder at the south side of the home which did not extend at least three (3) feet above the upper landing surface, exposing employees to a fall hazard of approximately 17 feet 6 inches. (b) On or about March 14, 2025, at 600 E. Quail Run, Oak Creek, WI 53154, employees were accessing the roof to conduct roofing work from a ladder at the south side of the home (garage side) which did not extend at least three (3) feet above the upper landing surface, exposing employees to a fall hazard of approximately 8 feet 3 inches.
