(a) On or about August 14, 2025, at W176 N7260 Raw Amber Ct, Menomonee Falls, WI 53051, employees installing a roof on a residential home roof with a 9:12 pitch were not using fall protection. This exposed employees to a fall hazard of approximately 26 feet 2 inches from the roof eve to the ground below on the back (North) side of the home. Christian Martinez DBA Christian Martinez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1811089, citation number 1, item number 2 and was affirmed as a final order on April 30, 2025, with respect to a workplace located at 600 E. Quail Run, Oak Creek, WI 53154.
