29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b): a) Employees performing roofing work are not protected from fall hazards by the use of a fall protection system. The employer fails to assure effective protections against falls as required by the standard, as demonstrated on March 18, 2025, at 385 Tallulah Avenue, Jacksonville, FL, when employees were performing roofing operations and were exposed to an approximate 10-foot and 3-inch fall hazard to the ground, in that; fall protection was not installed or utilized.

29 CFR 1904.41(a)(1)(i): The establishment had 20 or more employees but fewer than 250 employees at any time during the previous calendar year, and the establishment was classified in an industry listed in appendix A to subpart E of this part, and the employer did not electronically submit information from OSHA Form 300A Summary of Work-Related Injuries and Illnesses to OSHA or OSHA's designee by the specified date: a) Elo Restoration, LLC. located at 3415 Kori Road, Jacksonville, FL: On or about March 18, 2025, and times prior thereto, the employer failed for calendar year 2024, to electronically submit information from the OSHA Form 300A or equivalent by 03/02/2025.
