29 CFR 1910.159(c)(8)(iii): Sprinklers on automatic sprinkler systems were not protected from mechanical damage. First Floor Warehouse: On and/or before March 18, 2025, four fire suppression sprinkler heads were missing cages and/or guards in low-lying ceiling areas. Potential damage to sprinkle heads could prevent activation in the event of a fire and the exposed deflector could cause a laceration. This condition potentially exposed employee(s) to fire and/or laceration hazards.

29 CFR 1910.157(e)(2): Portable fire extinguishers were not visually inspected at least monthly: On/before March 18, 2025, two Class A fire extinguishers were not inspected monthly in the following locations: a. Upper Mezzanine near Double Doors: Last visually inspected on January 27, 2025. b. Cosmetic Cage near Stairs: Last visually inspected on January 6, 2025. Employees were exposed to smoke and burn related injuries from potential fire hazards.
