29 CFR  1926.102(a)(1): The employer did not ensure employees used eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   On or about March 21, 2025, employees were exposed to struck by hazards, while performing roofing work on a residential home located at 12164 Hilary DR NW, North Royalton Ohio 44133; the employer did not ensure employees working on the roof of a single family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material using hand tools and a leaf blower to remove material from the roofing surface.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about March 21, 2025, employees were exposed to fall hazards, while performing roofing work on a residential home located at 12164 Hilary DR NW, North Royalton Ohio 44133, the employer did not ensure employees working on a residential roof were protected from a fall of approximately 9.7 feet by utilizing a fall protection system.  Quality Roofing and Renovation LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to use fall protection), which was contained in OSHA inspection number 1790076, citation number 01, item number 01 and was affirmed as a final order on 2/8/2025, with respect to a workplace located at 4041 Chestnut RD, Seven Hills Ohio 44131.  Quality Roofing and Renovation LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to use fall protection), which was contained in OSHA inspection number 1786855, citation number 01, item number 01 and was affirmed as a final order on 2/8/2025, with respect to a workplace located at 6110 Dora Blvd, Independence Ohio 44131.  Quality Roofing and Renovation LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to use fall protection), which was contained in OSHA inspection number 1746671, citation number 01, item number 01 and was affirmed as a final order on 7/16/2024, with respect to a workplace located at 6461 Mandalay RD, Parma Heights Ohio 44130. 29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about March 21, 2025, employees were exposed to fall hazards, while performing roofing work on a residential home located next to 12164 Hilary DR NW, North Royalton Ohio 44133; during the installation of roofing material on a residential roof, the employer did not ensure portable ladders extended 3 feet above the upper landing surface.
