29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   The employer does not require the use of any form of fall protection during roofing work. This violation was most recently observed on June 26, 2025, at a residential construction project located at 7719 Goldenrod Drive, Mentor, OH 44060, where employee(s) were not protected from fall hazards approximately 9 feet to the ground. To abate this violation, the employer must ensure that its employees performing roofing work are protected from falls, and that no employee is allowed onto a roof more than six feet high without adequate fall protection.  Quality Roofing and Renovation LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to use fall protection), which was contained in OSHA inspection number 1812737, citation number 02, item number 01 and was affirmed as a final order on 5/6/2025, with respect to a workplace located at 12164 Hilary DR NW, North Royalton, OH 44133.  Quality Roofing and Renovation LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.501(b)(13) Duty to use fall protection), which was contained in OSHA inspection number 1790076, citation number 01, item number 01 and was affirmed as a final order on 2/8/2025, with respect to a workplace located at 4041 Chestnut RD, Seven Hills Ohio 44131. 29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about June 26, 2025, at 7719 Goldenrod Drive, Mentor, OH 44060, the employer failed to ensure employees utilizing a portable ladder to gain access to the roof to perform weatherproofing work were protected from a fall of approximately 9 feet in that the ladder did not extend 3 feet above the landing zone.
