29 CFR 1910.28(b)(13)(i): The employer did not ensure that when work is performed on low slope roofs, each employee is protected from falling by a guardrail system, safety net system, travel restraint system, or personal fall arrest system. a.) PJ Murphy Forest Products Corp., - On or about 4/22/25, and at times prior thereto, employees were engaged in work activities on the low slope roof without fall protection. Employee was working up to 14-feet above ground level. Work activities included, but were not limited to, repairs to the roof. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017. a.) PJ Murphy Forest Products Corp., - On or about 4/22/25, and at times prior thereto, employees were engaged in work activities on the roof without fall protection training. 29 CFR 1910.28(b)(1)(i):The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2m) or more above a lower level was protected from falling by guardrail systems, safety net system, or personal fall arrest system: a.) PJ Murphy Forest Products Corp., shaker area - On or about 10/28/24, and at times prior thereto, employees were engaged in work activities on an expanded metal platform without the use of fall protection. Employees were working up to 10-feet above ground level. Work activities included, but were not limited to, repairs to the shaker and cleaning dust accumulation off the platforms and stairs. On October 28, 2024, an employee fell from this area and sustained multiple fracture injuries. The PJ Murphy Forest Products Corp, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1910.28(b)(1)(i), which was contained in OSHA inspection number 1734018, citation number 1 item number 1a and was affirmed as a final order on 9/3/24, with respect to a workplace located at 2589 Campbelltown Road, Kane, PA 16735. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
