P J Murphy Forest Products, Corp - On or about 3/27/25,  employees were operating forklifts including but not limited to the Toyota Forklift without being trained, evaluated, and certified on the forklift. PJ Murphy Forest Product Corp, drum roller dryer - on or about 3/27/25, and at times prior thereto, the gate on the drum roller was not interlocked to ensure the drum roller would stop moving when the gate was opened.   b.) PJ Murphy Forest Product Corp., drum roller dryer -  on or about 3/27/25, and at times prior thereto, the back of the drum roller near the wall area was not guarded. 29 CFR 1910.29(b)(2): The employer did not ensure that guardrail systems met the requirement that  mid rails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members that are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high.   a.) PJ Murphy Forest Products Corp., outside near the dust collection system- on or about 3/27/25, and at times prior thereto: The stairway was not equipped with a mid rail.   b.) PJ Murphy Forest Products Corp., furnace area- on or about 3/27/25, and at times prior thereto: The stairway was not equipped with a mid rail. The stairway led from the furnace area up to the entrance.   The PJ Murphy Forest Products Corp, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR1910.29(b)(2), which was contained in OSHA inspection number 1734018, citation number 1 item number 2a and was affirmed as a final order on 9/3/24, with respect to a workplace located at 2589 Campbelltown Road, Kane, PA 16735.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). PJ Murphy Forest Products, Corp. drum roller area - On or about 3/27/25, and at times prior there to, the flexible metal cable assembly was pulled away from the electrical box leaving the wires exposed.  The electrical box was associated with thermostat for the temperature on the drum roller.
