29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):  At the job site located at 601 East Elmwood St. Park City, Kansas, employees engaged in residential roofing activities were exposed to fall hazards of approximately 10 feet 6 inches from the eave of the roof to the ground below in that fall protection was not utilized.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  At the job site located at 601 East Elmwood St. Park City, Kansas, employees engaged in residential roofing activities from a 4:12 pitch roof were exposed to fall hazards of approximately 10 feet 6 inches from the eave of the roof to the ground below in that the extension ladder used to access the roof was not extended at least 3 feet above the eave or was not secured to the structure.
