At a jobsite located at 10419 General Avenue, Jacksonville, FL: On or about April 7, 2025, the employer exposed employees to a struck-by hazard, in that restraining devices, such as tire cages were not equipped on each vehicle used for service calls.

At a jobsite located at 10419 General Avenue, Jacksonville, FL: On or about April 7, 2025, the employer exposed employees to a struck-by hazard, in that employees were allowed to fully inflate tires on multi-piece rims outside of a restraining device.
