29 CFR 1910.28(b)(1)(i):Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i): On or about March 30, 2025, an employee accessing a rubber jam at the Mixer 19 pendulum wigwag stacker was exposed to the hazard of falling approximately 12 feet when walking on and working from a cable tray. The employee was not protected from falling by personal fall protection system or other means. Continental Tire The Americas, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.28(b)(1)(i), which was contained in OSHA inspection number 1715807, citation number 1, item number 1 and was affirmed as a final order on January 2, 2025, with respect to a workplace located at 11525 North IL Hwy 142, Mount Vernon, IL 62864. 29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about March 30, 2025, employees were exposed to machine hazards associated with moving parts during servicing and unjamming of the Mixer 19 pendulum wigwag stacker. The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards. Continental Tire The Americas, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(d), which was contained in OSHA inspection number 1564841, citation number 2, item number 2 and was affirmed as a final order on February 22, 2023, with respect to a workplace located at 11525 North IL Hwy 142, Mount Vernon, IL 62864.

On or about March 30, 2025, a recordable injury occurred when an employee received fractured vertebra and ribs when they became caught under the Mixer 19 conveyor at the pendulum wigwag stacker resulting in lost time but no days were entered.
