29 CFR 1910.147(f)(4): Specific procedures were not utilized during shift or personnel changes to ensure the continuity of lockout or tagout protection, including provision for the orderly transfer of lockout or tagout device protection between off-going and oncoming employees, to minimize exposure to hazards from the unexpected energization or start-up of the machine or equipment, or the release of stored energy:    On or about April 10, 2025, following multiple shifts of service activities, an employee was assigned to perform service, including inspecting, on the Cure Press 02A Post-Cure Inflation machine and was exposed to the release of stored energy.  The employer failed to utilize specific procedures for shift or personnel changes that ensured the continuity of lockout or tagout protection between off-going and oncoming employees.

29 CFR  1910.147(c)(4)(ii): The procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A-D):  On or about April 24, 2025, the Tire Press CP02A procedure developed for the entirety of the machinery, including CP02A Post-Cure Inflation (PCI), failed to provide a specific statement of the intended use(s) of the procedure for servicing the CP02A or its PCI in accordance with 1910.147(c)(4)(ii)(A). The procedure also failed to clearly identify all specific steps to be followed by employees to: shut down; isolate all  hazardous pneumatic, steam, and thermal energy sources in accordance with 1910.147(c)(4)(ii)(B). The procedures also lacked clear and specific requirements for testing the CP02A or its PCI to determine and verify the effectiveness of the lockout devices and energy control measures in accordance with 1910.147(c)(4)(ii)(D).  Continental Tire The Americas, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1715807, citation number 1, item number 2 and was affirmed as a final order on January 2, 2025, with respect to a workplace located at 11525 North IL Hwy 142, Mount Vernon, IL 62864.  Continental Tire The Americas, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1564841, citation number 1, item number 1 and was affirmed as a final order on February 22, 2023, with respect to a workplace located at 11525 North IL Hwy 142, Mount Vernon, IL 62864.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about April 10, 2025, employees were exposed to machine hazards associated with moving parts and stored energy during servicing of the Cure Press 02A Post-Cure Inflation machine.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.
