29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On or about April 23, 2025, at the worksite, Lot 2; Bishop Brooks Drive, Manlius, NY 13104: The employer failed to ensure the side rails of a portable ladder employees were using to access a second story roof had the side rails extended 3 feet above the access point. Phil Cousineau dba PVC Exteriors, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(1), which was contained in OSHA inspection number 1659024, citation number 2, item number 2 and was affirmed as a final order on September 12, 2023, with respect to a workplace located at Lot# 179 Austin Meadows, Manlius, NY.
