29 CFR  1903.19(c)(1): The employer did not certify to OSHA, within 10 calendar days after the abatement date, that the cited violation had been abated:    (a) EDD Services LLC failed to certify abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected:  Citation 1, Item 3,  a serious violation of 29 CFR 1926.501(b)(15) with an abatement date of September 26, 2025.    Citation 1, Item 4 a serious violation of 29 CFR 1926.502(d)(17) with an abatement date of September 26, 2025.    Citation 1, Item 5  a serious violation of 29 CFR 1926.1053.(b)(1) with an abatement date of September 26, 2025.

(a) EDD Services failed to document abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected:  Citation 1, Item 2, a serious violation of 29 CFR 1926.501(b)(13) with an abatement date of October 24, 2025.    Citation 1, Item 2 a serious violation of 29 CFR 1926.1053(b)(1) with an abatement date of October 9, 2025.

Burrell St, Oak Creek, WI, the employer did not ensure employees performing roofing work on a residential construction site with pneumatic nail guns were utilizing safety glasses, exposing employees to eye injuries.

Burrell St, Oak Creek, WI, the employer had not ensured that employees laying shingles a residential property were protected from fall hazards therefore, exposing the employees to a fall hazard of approximately 10 feet to the ground.

29 CFR  1926.501(b)(15): Each employee on a walking/working surface 6 feet (1.8 meters) or more above lower levels was not protected from falling by a guardrail system, safety net system, or personal fall arrest system:   (a) On or about April 28, 9691 S. Burrell St, Oak Creek, WI, employees assisting with roofing activities while working from an Equipter RB4000 elevated dump trailer were not protected from falling to the lower level by guardrails or a personal fall arrest system. This exposed employees to a fall hazard of approximately 12 feet or more onto grass and equipment.

The attachment point of the body narness was not located in the center of the wearer's back near shoulder level, or above the wearer's head.  (a) On or about April 28, 2025 at 9691 S. Burrell St, Oak Creek, WI the employer had not ensured the attachment point of the body harness was located in the center of the wearer's back by clipping it to the D-Ring on the harness, while performing roofing work on an existing residential home. This exposed employees to a approximately 10 feet fall.

29 CFR  1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support.  (a) On or about April 28, 2025 at 9691 S. Burrell St, Oak Creek, WI, the employer did not ensure the Articulating Ladder used by employees to access and egress the residential roof on the front of the home was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 10 feet to the ground below.  (b) On or about April 28, 2025 at 9691 S. Burrell St, Oak Creek, WI the employer did not ensure the Articulating Ladder used by employees to access and egress the residential roof on the south end of the home was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 10 feet to the ground below  (c) On or about April 28, 2025 at 9691 S. Burrell St, Oak Creek, WI,  the employer did not ensure the Articulating Ladder used by employees to access and egress the residential roof on the north end of the home was extended at least three feet above the upper landing.
