29 CFR  1903.19(d)(1):The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a) EDD Services LLC, failed to document abatement within 10 calendar days after the abatement date that each of the following cited violations had been corrected:  Citation 1, Item 1, a serious violation of 29 CFR 1926.1053(b)(1) with an abatement date of October 7, 2025.  Citation 2, Item 1, a repeat serious violation of 29 CFR 1926.501(b)(13) with an abatement date of October 7, 2025.  In accordance with 29 CFR 1903.19(d)(1), abatement certification and documentation of abatement is required for this violation (using the CERTIFICATIONS OF THE CORRECTIVE ACTION WORKSHEET).

29 CFR  1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) On or about June 4, 2025 at 3848 Hazelbranch Lane, Oak Creek, WI 53154, the employer did not ensure the extension ladder used by employees to access and egress the residential roof was extended at least three feet above the upper landing. Exposing employees to a fall hazard of approximately 9 feet to the ground below.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about June 4, 2025, at 3848 Hazelbranch Lane, Oak Creek, WI 53154,  employees repairing shingles on the  front of the residential home steep roof were not using fall protection. This exposed the employees to a fall hazard of approximately 10 feet from the roof eave to the ground below.  (b)  On or about June 4, 2025, at 3848 Hazelbranch Lane, Oak Creek, WI 53154,  employees repairing shingles on back of the residential home steep roof were not using fall protection. This exposed the employees to a fall hazard of approximately 10 feet from the roof eave to the ground below.  The EDD Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.501(b)(13), which was contained in OSHA inspection number1809537, citation number 1, item number 1 and was affirmed as a final order on 6-9-2025, with respect to a workplace located at Units 9,10,11,12 N Bay Rd, Palos Heights, IL 60463.
