29 CFR  1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets.  (a) On or about May 26, 2026, at 954-960 Autumn Ridge Lane, Hartford, WI 53027, the employer did not ensure the employees assisting in roofing operations (ground workers) were protected from potential falling objects from above by the use of a protective helmet. This exposed the employees to a struck-by hazard from objects falling from a height of up to approximately 19 feet 7 inches on the back side of the home.  (b) On or about May 26, 2026, at 954-960 Autumn Ridge Lane, Hartford, WI 53027, the employer did not ensure the employees assisting in roofing operations (ground workers) were protected from potential falling objects from above by the use of a protective helmet. This exposed the employees to a struck-by hazard from objects falling from a height of up to approximately 19 feet 4 inches on the right side of the home.

(a) On or about May 26, 2026, at 954-960 Autumn Ridge Lane, Hartford, WI 53027, employees installing roofing were not using fall protection. This exposed the employees to a fall hazard of approximately 19 feet 7inches from the eave to the ground below on the back side of the home. (a) On or about May 26, 2026, at 954-960 Autumn Ridge Lane, Hartford, WI 53027, an employee conducting roofing activities on a 7:12 pitch roof was not properly protected from falling to the lower level. The attachment point of the body harness was on the waist belt and not located in the center of the wearer's back near shoulder level, or above the wearer's head as required. This exposed the employee to severe spinal and neck injuries.  EDD Services LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR  1926.502(d)(17), which was contained in OSHA inspection number 1821191, citation number 1, item number 4, and was affirmed as a final order on November 26, 2025, with respect to a workplace located at 9691 S.
