29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: Workshop: On or about May 14, 2025, and at times prior thereto, employees used an abrasive wheel bench grinder (Craftsman Serial No. 0226-W) with the work rest not adjusted to 1/8th of an inch. The distance measured from the abrasive wheel was 1/2 of an inch. Employees were exposed to face injuries.

29 CFR 1910.157(e)(3): Portable fire extinguishers were not subjected to an annual maintenance check: Office : On or about May 14, 2025, and at times prior thereto, the employer did not assure that all portable fire extinguishers are subjected to an annual maintenance check, such as, but not limited to, the extinguishers last inspected in April 2024. The USDA Forest Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(e)(3), which was contained in OSHA inspection number 1469479, citation number 02, item number 01, and was affirmed as a final order on 05/26/2020, with respect to a workplace located at Mi-Wok Ranger District24695 Hwy 108, Mi Wuk Village, CA 95346
